ISP Individual Support Plan--plan used with adults participating in vocational and/or residential programs.
LTD Long Term Diversion
medical card "Medical card" refers to the 11 1/2" white and green paper that the state sends monthly to individuals who are enrolled in the Oregon Health Plan. The medical card states the kind of benefits for which the individual is eligible, other insurance, social security number and other information.
